Medical cannabis for chronic back pain or intractable pain

DO I QUALIFY ?

Listed below are qualifying medical conditions per Minnesota law.

  • Cancer associated with severe/chronic pain, nausea or severe vomiting, or cachexia or severe wasting

  • Glaucoma

  • HIV/AIDS

  • Tourette syndrome

  • Amyotrophic lateral sclerosis (ALS)

  • Seizures, including those characteristic of epilepsy

  • Severe and persistent muscle spasms, including those characteristic of multiple sclerosis

  • Inflammatory bowel disease, including Crohn’s disease

  • Intractable pain

  • Irritable Bowel Syndrome (Effective 8/2023)

  • Obsessive Compulsive Disorder (Effective 8/2023)

  • Post-traumatic stress disorder

  • Autism spectrum disorder (must meet DSM-5)

  • Obstructive sleep apnea

  • Alzheimer's disease

  • Chronic pain

  • Sickle cell disease

  • Chronic motor or vocal tic disorder

HOW IT WORKS

STEP 1: VISIT WITH A LICENSED PROVIDER

Consult with our medical marijuana certification providers who will evaluate your qualifying condition and answer any questions you have about the MN Medical Marijuana program. The initial assessment is a face to face meeting per MN State regulation.

After identifying your qualifying condition, our health care practitioner will go online to the Patient Registry to certify you.

Give your practitioner an e-mail address appropriate for MMJ communications. Use a personal email that others don’t have access to. Please fill, sign and upload the Patient Email and Acknowledgement Form (Click here for form) or you can find this in your patient portal once your appointment is confirmed.

STEP 2: GET APPROVED

After your practitioner has certified that you have a qualifying condition, you will receive an email from the Office of MMJ. (this will be the email address you provided on file)

The email will contain the link you need in order to register online and an important information sheet you should review before completing registration.

STEP 3: Register/Enroll Online

You will need the following:

  • The link in your email from the MN office of Medical Marijuana.

  • Your name, address and phone number.

  • Government-issued ID: state ID, driver’s license, or passport (scan or photo from phone).

The payment must be received within 90days of certification.

STEP 4: Receive Approval Email

You will receive an email from the office of MMJ when your account has been approved.

Once you receive the approval email, please complete the patient self-evaluation report to expedite your visit to the cannabis patient center. You should then schedule an appointment and visit a dispensary. Click here for locations

PS: If you are a veteran, effective July 1st 2024 you will be able to use a separate application system to enroll (self -certify) for Medical Cannabis.
